When you’re being discharged from hospital having somewhere safe to call home is important.

We can work with you while you're in hospital to help make sure that you have a safe place to go when you're discharged.

Caerphilly

We provide housing related support and advice to people who may be homeless/at risk of homelessness with mental health needs through a Community Intervention Worker, Hospital to Home Worker and Mental Health Link Worker.

The Mental Health and Housing Support Worker will provide direct housing advice and support to ensure a recovery orientated mental health service.

Newport

Our staff are based at St Cadoc’s Hospital providing short-term housing related support and advice to in-patients. This service provides assessment and a housing health check prior to discharge.
