Hospital to home

When you’re being discharged from hospital having somewhere safe to call home is important.

We can work with you while you’re in hospital to help make sure that you have a safe place to go when you’re discharged.

We provide these services in the following areas:

Caerphilly

We provide housing related support and advice to people who may be homeless/at risk of homelessness with mental health needs through a Community Intervention Worker, Hospital to Home Worker and Mental Health Link Worker.

The Mental Health and Housing Support Worker will provide direct housing advice and support to ensure a recovery orientated mental health service.

Newport

Our staff are based at St Cadoc’s Hospital providing short-term housing related support and advice to in-patients. This service provides assessment and a housing health check prior to discharge.
